Question: Who, besides the head coach of each sport, is responsible for monitoring APR?

Dir of Ops for sport(as help to HC, Academic Services for Athletes (Head + Head of Sport(if app)), Sport Admin. (Asst/Aosc AD with primary oversite).
It is a combo of the above people. There are more than enough people to look after them.

and ultimately when the first inkling of a problem was discovered the AD an Chancellor should have been given timely updates and acted on them. In this case it seems that in fact there were not enough people looking after it although I understand what you are saying.
